The Children's Long-Term Support (CLTS)Waiver Program is a Home and Community-Based Service (HCBS) Waiver that provides Medicaid funding for children who have substantial limitations in their daily activities and need support to remain in their home or community.
Wisconsin’s CLTS Program supports children with disabilities and their families by providing services that help children grow and live their best lives in their home and community. Each child and family works with a support and service coordinator to develop a plan to meet their goals. Funding can be used to access a range of services based on an assessment.
Eligibility:
Under age 22
Wisconsin resident
United States citizen or acceptable immigration status
Long-term disability, such as a developmental, physical, and/or severe mental health condition
Live in a home or community setting
