License renewal fees for the Environmental Health program can be paid by check to the Health Department address or by credit card using the AllPaid Payment Services.

Flu Clinic Information:
Regular Quadrivalent, Preservative-Free Flu Shots cost $35.
High-Dose Quadrivalent, Preservative-Free Flu Shots (65+ years) cost $85.

Insurances that are accepted are:
Medicare & Supplemental Insurance, Medicaid/Badgercare, HealthPartners, Group Health (may or may not be covered ask clinic staff), Security Health, United Health Care, and Managed Health Services

Disclaimer: If we are unable to bill your insurance, or are not presented with an insurance card upon registration you will be billed the full or remainder of the cost of the vaccine.
